Is there a food plan/chart for type II hypertensive diabetes?

My son was recently diagnosed with type II diabetes and hypertension. Is there a food plan/chart that he should be following in order to keep him as healthy as possible?

While no particular eating plan is the best, a good eating plan will include whole grains, minimally processed foods, with fibre-rich fruits and vegetables. It will also contain fewer carbs, lean protein, healthy fats, and less processed sugars. You can work with your son to draft an eating plan using this.
